When an In-Person Visit May Be Required
Telemedicine is not appropriate for every condition. An office visit, urgent care evaluation, or emergency treatment may be recommended for:
  • Severe infection
  • Rapidly spreading redness or swelling
  • Open wounds requiring hands-on treatment
  • Suspected fractures
  • Circulation problems requiring testing
  • Significant deformity or instability
  • Conditions requiring imaging, procedures, or surgery

Patient safety always takes priority over virtual convenience.

Covered neuropathy management visits are billed to Medicare or your insurance per visit, not sold as a prepaid package. Any non-covered wellness or supplement services are reviewed with you in advance. An Initial Consultation is required first.
How the Plans Compare

Payment: Initial Consultation collected at booking by phone. Annual plans can be paid in full or in monthly installments. Annual plans are non-refundable after the first visit; unused visits expire at the end of the 12-month period.
Insurance & Payment
Advanced Foot Care Center is enrolled with Medicare. For telemedicine and in-office visits, here's how billing works:
Insurance-Covered Telemedicine
If your insurance covers telemedicine, we can provide covered medical services through your plan and bill your insurance directly. Please note that wellness visits and wellness plans are not covered by insurance — these are billed separately as a cash-pay service.
Costs Reviewed in Advance
Some services (for example, certain laser or wellness consultations) may not be covered. If that applies, we'll explain the service, give you a cost estimate, and review your options before the service is provided — no surprises.
Medical-Necessity Support
When appropriate, Dr. David Velarde, DPM can provide documentation of medical necessity to support a covered claim.
Patient Responsibility
You are responsible for any copays, deductibles, coinsurance, or services determined not to be covered, after we've reviewed those costs with you.

Important Disclosures
Telemedicine consultations are available only to patients physically located in Tennessee at the time of the visit. Dr. Velarde is licensed to practice podiatric medicine in Tennessee. Visits are conducted via HIPAA-compliant secure video and your health information is never shared without your consent.
In-Person Visits May Be Required
Some conditions may require an in-person visit for physical examination, imaging, or treatment. Dr. Velarde will advise you if that is the case.
Prescription Medications
Prescription medications may be issued when medically appropriate and permitted under Tennessee law. Prescription decisions are based solely on clinical judgment and are not guaranteed as part of a telemedicine consultation.
Telemedicine and Wound Care
Certain wound conditions may be appropriate for telemedicine follow-up, review of photographs, care coordination, patient education, and monitoring. However, many wounds require in-person examination, vascular testing, imaging studies, debridement, or urgent medical treatment. If an in-person evaluation is necessary, recommendations will be provided during your consultation.
How Payment Works
Wellness visits and wellness plans are a cash-pay service, with payment due at the time of visit. Covered medical services may be billed to Medicare or your insurance — see Insurance & Payment for details.
No Physician-Patient Relationship Until Evaluation
This page does not establish a physician-patient relationship until you are formally evaluated and accepted into care by Dr. Velarde.
